Schedule an appointment to talk to Dr. Cheryl using your cell phone or computer. If she thinks cannabis can help, you will discuss treatment options and you will receive your certification form. You will then need to finish the registration process with the state.

Once Dr. Cheryl has diagnosed you with a qualifying condition (see list below) and you have received your certification, you will also need to register with the Medical Marijuana program through the state’s online Patient Registration System.

Once you’ve been approved by the state of Iowa, you will receive an email with a temporary card to purchase medical marijuana immediately. The hard copy of the card will be mailed to you in aroind 2-4 weeks.
Qualifying Conditions
An Iowa resident may qualify for Iowa's Medical Cannabidiol Program if their Healthcare Practitioner certifies they have one of the following Qualifying Debilitating Medical Conditions:
- Chronic Pain
- Cancer – if the illness or its treatment produces one or more of the following: severe or chronic pain, nausea or severe vomiting, cachexia or severe wasting.
- Seizures
- Crohn’s disease
- Multiple Sclerosis with severe and persistent muscle spasms
- AIDS or HIV (as defined in Iowa Code, section 141A.1)
- Amyotrophic lateral sclerosis (ALS)
- Parkinson’s disease
- Post-Traumatic Stress Disorder (PTSD)
- Any terminal illness with a probable life expectancy of under one year
- Ulcerative colitis
- Severe, intractable pediatric autism with self-injurious or aggressive behaviors
- Severe, intractable autism with self-injurious or aggressive behaviors
- Corticobasal Degeneration
