Schedule an appointment to talk to Dr. Cheryl using your cell phone or computer. If she thinks cannabis can help, you will discuss treatment options and you will receive your certification.

Once Dr. Cheryl has diagnosed you with a qualifying condition (see list below) and you have received your recommendation, Dr, Cheryl will input your information into the patient registry. You will receive an email from the state of Ohio to finish your registry activation. You will have to pay an additional state fee of $50.

Once you’ve been approved by Dr. Cheryl, your physical card will arrive within 2 weeks of registration with the state, pending approval. You will then be able to purchase medical marijuana.
Qualifying Conditions
Under Ohio law, qualifying medical conditions for medical marijuana include all of the following:
· Chronic, Sever, or Intractible Pain
· AIDS
· Amyotrophic lateral sclerosis
· Alzheimer’s disease
· Cachexia
· Cancer
· Chronic traumatic encephalopathy
· Crohn’s disease
· Epilepsy or another seizure disorder
· Fibromyalgia
· Glaucoma
· Hepatitis C
· Huntington’s disease
· Inflammatory bowel disease
· Multiple sclerosis
· Parkinson’s disease
· Positive status for HIV
· Post-traumatic stress disorder
· Sickle cell anemia
· Spasticity
· Spinal cord disease or injury
· Terminal illness,
· Tourette syndrome,
· Traumatic brain injury
· Ulcerative colitis
